Disability/FMLA accommodation consultations are available for patients who have been under the care of Mendem Health for at least six months before the request. Schedule an accommodation consultation appointment to discuss criteria and options for accommodation. Patients who seek accommodation must have a primary doctor and actively attend weekly therapy sessions. Contact our office for more details.
IEP and 504 Plan
Our provider is willing to work in collaboration with your teen’s PCP, therapist, and school support team to develop the appropriate treatment plan and accommodations. There are strict conditions for which this request can be met. Please discuss this option with your provider during your session.
Emotional Support Animal Letters
Our office does not provide ESA letters. Under the Americans with Disabilities Act, “Emotional support animals, comfort animals, and therapy dogs are not service animals under Title II and Title III of the ADA. Other species of animals, whether wild or domestic, trained or untrained, are not considered service animals either. The work or tasks performed by a service animal must be directly related to the individual’s disability. It does not matter if a person has a note from a doctor that states that the person has a disability and needs to have the animal for emotional support. A doctor’s letter does not turn an animal into a service animal.” If a service animal is needed, consult with your provider regarding options for this type of certification.
