Mendem Health provides high-quality mental health services via telehealth using an evidence-based approach. We understand each person is unique and has individual needs and therefore we offer individualized treatment plans. We offer a holistic, integrated approach to providing culturally competent care.
Please request an appointment via the purple Message Us Now link on the top of our website. Once we have accepted your request, you will be scheduled for your first appointment, and our staff will help you with the onboarding process.

Step One: Message Us and Request an Appointment
Click the purple Message Us button on the bottom let screen to request an appointment. Just answer a few short questions so we can understand your needs. Once your appointment request is approved, our office will email you a link to schedule an appointment and register your Patient Portal access. On the patient portal you will upload your photo ID and insurance cards. If the patient is a minor, please include a photo of the teen.
Step Two: Complete your Consent Forms
In order to hold your scheduled appointment your Consent Forms must be completed within 24-hours of scheduling your appointment.
Step Three: Complete your Intake Forms
Your Intake Forms give the provider some background information to your condition. During the session, you will review the form with your provider. Please have this form completed before your appointment to avoid any session delays.
