FAQ: Mendem Simple

Cancellation and Late Policy: We understand that life can get in the way and may prevent you from attending your scheduled session, but appointments are in high demand and our goal is to accommodate those who are in need of our care. We allow patients to give 48 business hours’ notice of cancellation to avoid any fees.

If you are more than 15 minutes late to your scheduled appointment without notifying the office, you will need to reschedule. This is considered a missed appointment and will be charged a cancellation fee. In cases of emergency, the cancellation fee may be waived with supporting documents.

Cancellation Fees: Our cancellation fee is $175 and due at the time of cancellation. Patient will not be allowed to reschedule an appointment until the fee is paid in full. If this is your initial appointment, the provider reserves the right to accept or deny future services.

Self -Pay Rates for Mendem Simple:

Initial Assessments: 45 minutes $250
Follow up: 30 min $150
Med Check follow up: 15 minutes $75

Insured Patients:

We are in network with most major insurance carriers to include Aetna, Blue Cross Blue Shield, Carefirst, Cigna, United Healthcare, Optum, and Maryland Medicaid. Patients should check with their plan to make sure our provider is in their plan network. Patients are also responsible for understanding their mental health benefits, deductible responsibilities, and copays.

Attending scheduled follow-up appointments is important to make sure your medications are filled in a timely manner. There is a $25 convenience fee for medication refill requests. Requests are filled at the provider’s discretion and based on your clinical status. Requests are filled during the normal business hours of Monday-Friday 9 am-5 pm, with the exception of office closures during holidays.

At this time, we only provide televisits for patients who reside in the District of Columbia, Maryland, and Virginia.

Providing care outside of these jurisdictions is illegal and compromises the provider’s licensure. If it is discovered that you are out of the jurisdiction for the session, your session will be terminated immediately and you will be charged the cancellation fee. The provider reserves the right to dismiss you from Mendem’s services due to liability risks.

All patients can request a copy of their records free of charge. Records requests are completed between 15-30 business days from the date of the request. Documents sent to other providers or facilities are free of charge unless the party is requesting another form of delivery (i.e., paper chart, thumb drive, etc.)

There are laws put in place to protect the privacy of your teen’s medical information around reproductive health, mental illness, and substance abuse. Please read the attached document for a clear understanding of the limitations of our office releasing certain information.
