Appointment Requests

Our practice is focused on the care of adolescent girls and women. We specialize in treating anxiety, depression, disordered eating, and perinatal and postnatal anxiety and mood disorders. All sessions are conducted via secure, HIPAA-compliant technology.

New Patients

Please complete the Request New Patient prescreen form to request an appointment. Within one business day, we will respond with acceptance or denial of your request for a new patient consultation. During your appointment, we will review your intake form and complete a comprehensive assessment of your current condition. Once your appointment is accepted, you will be responsible for signing your consent forms and completing your intake forms before your appointment via the patient portal. During your initial session, you and your provider will review your intake forms and discuss your issues of concern. During this time, your provider may order labs, require a review of records from previous providers, or further diagnostic testing. Follow-up will be determined between you and the provider based on your treatment planning.

Existing Patients

Please register your patient account in the patient portal. In the patient portal, you can self-schedule, review labs, request medication refills, send your records to other providers free of charge, message the office, and pay your bill. All patients must have access to the patient portal and utilize the patient portal for all services.

Important Note About Payments

We accept all major credit cards for payment. All clients will access their Patient Portal to make payments on outstanding balances and track payment history. Please have your credit card available before your appointment. Before all televisits, you will be prompted to make payments associated with your visit via a secure digital payment processor. All payments are due at the time of service, including copays, deductibles, and any outstanding balance.

Outstanding Balance Settlement

If you were an existing patient prior to 2023 with an outstanding balance, please contact our office for details regarding your balance. You may use the link below to settle your balance in increments or in full. Your outstanding balance must be paid in full prior to utilizing our services.
